Social Worker/Site Coordinator – Turk/Eddy Street

THE POSITION

Under the general direction of the Social Worker Manager, Service Coordinator/Site Coordinator is responsible for assisting residents living in TNDC properties with housing stabilization & retention by providing counseling and support services, community outreach, advocacy, information, and referrals to existing off-site providers. The Social Worker/Site Coordinator is also responsible for the supervision of off-site Social Workers. The position is full-time (40 hours per week), exempt.

KEY RESPONSIBILITIES

  1. Coordinate a service delivery program for tenants.
  2. Provide supervision to off-site Social Work Staff.
  3. Hire, orient and train new supervisees.
  4. Manage a caseload of individuals.
  5. Develop a strong working relationship with the on-site building management staff.
    Negotiate service contracts with off-site providers as necessary.
  6. Compile monthly statistics and monthly reports.
  7. Develop a directory of service providers to be used for staff and residents.
  8. Attend all required meetings and adhere to all reporting requirements.
  9. Identify residents in need of services and maintain awareness of residents’ health status; including but not limited to specific medical problems and mental health issues.
  10. Establish a care plan for each individual and family to ensure that they are setting realistic and achievable goals.
  11. Serve as liaison/advocate with TNDC property management, as well as outside social, health, financial benefits, training and other service agencies.
  12. Provide assessments for all individuals and families on caseload.
    Provide information and referrals to off-site service providers.
  13. Provide crisis intervention, conflict resolution and low intensity counseling to individuals.
  14. Advocate for clients to empower them to meet their own needs, when possible.
    Comply with all protective services reporting procedures.
  15. Develop new resources and relationships with outside service providers.
    Organize community-building activities and attend monthly community meetings in partnership with tenants and building management.
  16. Maintain accurate and up-to-date records, files, and statistics on individuals and/or families being served and clients receiving services on caseload.
  17. Facilitate and monitor resident referral system between building managers and support services.
  18. Provide 24-hour, on-call support to buildings on a rotating basis and 24-hour, on-call support to supervisees.
  19. Other duties as assigned.

MINIMUM QUALIFICATIONS

  • Master’s Degree in Social Work or a related field. A Bachelor’s Degree in Social Work or a related field, along with five or more years of experience overseeing and/or supporting the work of others may substitute for a Master’s Degree.
  • Two years experience working with very low-income people in a social service setting.
  • Two years of experience working with elderly populations.
  • One year of experience providing supervision to staff.
  • Demonstrated knowledge and understanding of substance abuse and mental health issues and their effects on individuals, families, elderly and immigrant populations.
  • Demonstrated knowledge and understanding of issues related to aging in place.
  • Knowledge of current resources and service/benefits programs in San Francisco and the Greater Bay Area.

PREFERRED QUALIFICATIONS

  • Bilingual: Korean/English
